What is a Photo Release Form?
Our Archdiocese requires that in order to use photographs of people (parishioners or not) in our print or online materials, consent must be obtained through a photo release form.
Do I need to fill out a form? If you have been asked by a SEAS program leader or SEAS employee to submit this form (and you are granting permission for your photo to be used in our print and/or online material), then yes.
If you are submitting a photo for a Sacrament, New Parishioner or Military bulletin board at SEAS, you do NOT need a photo release. These are displayed in-house only.
Click the appropriate form below to download and print a photo release, then sign and return it to us as soon as possible. A photo of the signed form is acceptable. Forms are good for one year and can be emailed to your SEAS contact or to info@seasparish.org.
